How do I create a new waiver form?

Creating new waiver forms is simple and just takes a couple of minutes.
  1. Head over to Forms Manager from the welcome screen or the menu
  2. Click the create a new waiver button
  3. From the popup window, give your waiver form a nickname and then click create waiver
    1. This name is for your internal use and it will not be visible by your participants
  4. After clicking create waiver you'll be taken directly into the waiver builder. From the sidebar you can configure your waiver's default settings:
    1. Participants (adults & minors, adults only, minors only)
    2. Enforce age of majority and minimum age for participants (optional)
    3. Set the lifespan of your waiver (from one-time use to never expire)
  5. Once you've set the defaults, you can then customize or replace the default waiver text
  6. Add fields to collect the information you want by clicking the ⊕ in the canvas:
    1. I Agree checkboxes
    2. Dropdown lists with optional tagged answer alerts
    3. Unlimited text fields
    4. Signatures or initials
    5. Photo ID uploads
    6. Groups
  7. Move and change the order of fields by dragging the handle at the top left of each one (it looks like a small grid )
  8. For advanced users, modules and groups of modules can be shown conditionally based on a single response. To use conditional logic, click on the module or group, then in the sidebar locate Show Field, and change AlwaysIf and choose a trigger:
    1. Activity name
    2. Checkbox
    3. List selection
    4. Adult participating
    5. Minor participating 
  9. Customize the information you want to collect from adult and minor participants by adding or removing fields to their respective group.
    1. If your waiver default is set to adults or minors, the option to add an unlimited number of minors is shown when the adult participant selects "I am an adult signing for a minor". 
  10. At the bottom, click the Consent and Submit box to open its settings. Here you can customize the digital signature consent agreement, enable a marketing opt-in question, and customize the labels for signing and submitting to match your operations.  
    1. You don’t have to manually add a signature field. A permanent signature field is included in submit settings, and every waiver must be signed before it can be submitted.
  11. If you offer different activities and you would like to know what each waiver is for, from the waiver settings tab, where you can configure an event or activity selector in the waiver so participants can select:
    1. Activities such as: Pool / Climbing Wall / Archery
    2. Services such as: Tattoo / Piercing
    3. Statuses such as: Member / Non-Member
      You can use conditional logic to show different waiver text and fields using these selections.
  12. Click the admin settings tab to customize how your waiver should function:
    1. Require approval
    2. Send a copy to the signer
    3. Redirect the signer to a website (perfect for FAQ's or return visit discounts)
    4. Add an optional confirmation QR code that your staff will scan from the waiver hub to verify the waiver.
  13. Optionally add your logo and personalize colors in your waiver from the branding & logo tab
  14. Rename labels or translate your waiver's functions from the translate & labels tab
  15. Click the Preview button at the top right to view your waiver as your guests will, and then make any adjustments.
  16. Save your changes and  you are ready to use your waiver. 
  17. When you're ready to share your waiver, access the share tab.
    1. From here you can customize your waiver's url to match your brand
    2. Use the Waiver Link in emails, on your website or in social media
    3. Use the Kiosk Link in a tablet to setup a universal Waiver Station Kiosk
    4. Download and print your waiver's QR code
👏 You are now ready to begin collecting waivers.
